The influence of eye yoga on vascular regulation and neuroplasticity in vision loss
Laufzeit: 01.05.2020 bis 30.09.2023

In glaucoma which leads to neuro-visual damage of retina, optic nerve or brain, vision can be partially restored by rehabilitation, but underlying neurovascular plasticity mechanisms are unclear.
Because long-term mental stress is a main cause of glaucoma, we conducted a randomized, controlled trial to study if relaxation using eye-yoga exercises combined with breathing meditation can improve visual field and eye movement dysfunction and normalize the typical vascular dysregulation.

While regarding the visual field analysis no significant improvements were detected in controls, vision recovery was observed in eye-yoga patients (p=0.001).

An interim analysis suggests that relaxation induced by eye-yoga and meditation is helpful to recover visual field loss and neurovascular regulation. The final conclusion need to wait the RCT completion.

Vascular dysregulation in glaucoma: retinal vasoconstriction and normal neurovascular coupling in altitudinal visual field defects

Purpose: Vascular dysregulation (VD) is a major factor in glaucomatous visual defect progression. However, little is known if neurovascular coupling (NVC) is impaired in glaucomatous retinal vessels and how it relates to vessel morphology and altitudinal visual field defect depth. To better predict the glaucomatous visual defect progression and possible vision restoration and establish personalized intervention, we need to further study the role of VD and NVC in glaucoma.

Methods: Using a dynamic vessel analyzer (DVA) we quantified retinal vessel diameters and dilation responses following neuronal activation by flickering light stimulation in primary open angle glaucoma (POAG) patients (n=30) and age-matched, healthy controls (n=22). Vessel dilation dynamics was measured as a function of vessel branch level (diameter) and degree of visual field impairment.

Results: In larger blood vessels average arterial and venous retinal vessel diameter was significantly smaller in glaucoma. However, when inducing neuronal activity by exposing the retina to flickering light, both arterial and venous dilation reached normal values despite having smaller diameters. This was largely independent of visual field depth.

Conclusions: Because dilation/constriction is normal, VD in glaucoma cannot be explained by impaired NVC but is rather caused by vasoconstriction. This may permanently limit energy supply to retinal (and brain) neurons and, depending on the extent of deprivation, lead to either long-term hypo-metabolic, surviving "silent” neurons or to cell death. The results we found will help with the establishment of predictive model based on VD in glaucoma progression and possible restoration in follow-up studies.

Treatment of patients with optic nerve damage with electrical stimulation: a home stimulation study

The overall aim of the current project is to investigate the efficacy and safety of long-term treatment of glaucoma by transorbital alternating current stimulation with a home-stimulation device (SASm). We hypothesize that tACS home-stimulation significantly improves vision and associated biomarkers compared to sham-controls. Our secondary aim is exploratory, namely to investigate possible mechanisms of action. The current study will be a double blind, parallel group interventional study. Participants (N=45) will be randomized to one of two intervention arms following a 2:1 scheme: (1) arm 1 - intervention group receiving tACS; (2) arm 2 - placebo group receiving sham (no tACS, only phosphene threshold measured at study entry). The primary endpoint of this clinical investigation is improved near-threshold Humphrey Visual Field Index of the worse eye/solely available eye following active tACS stimulation compared to sham, as indicated by relative change over baseline at end of treatment.

According to our goal of investigating the long-term tACS efficiency, participants will undergo 30 stimulation sessions. Endpoints will be assessed before, immediately after and 2 and 6 months after the end of treatment. To our knowledge, this would be the longest stimulation duration and follow-up period investigating the effect of tACS. It would allow us not only to collect information on long-term effects, but also to compare it with short-term interventions (i.e., 10 days). The study will be considered completed after the last follow-up measurement of the last patient.

To examine the retinal blood vessel dynamics, we want to use a CE-approved "Dynamic Vessel Analyzer" in an additional "open label" feasibility study on a sample of patients (N=20) from an outpatient care center to determine the effect of electrical stimulation on the blood supply. These patients will undergo 10 electrostimulation sessions using a device which is equivalent to the devices used for the other two-arms. As part of our study we will have two measurement points to investigate the effect of the electrical stimulation on the blood supply, namely before and after therapy.

Microsaccades in normal vision and in glaucoma and its treatment with eye movement training

Microsaccades are fast, jerk-like eye movements that happen once or twice per second. They are profoundly involved in visual perception. Microsaccades show also high clinical relevance e.g. alterations of microsaccades can cause symptoms such as diplopia, reduced visual acuity and blurred vision, which are reported in a series of ophthalmological and neurological diseases. The study addresses if microsaccade and microsaccade-related potentials are stable in normal aging. This explores the usefulness of microsaccades as a potential biomarker to monitor and better understand di erent diseases with oculomotor symptoms.

Spacetime in the Brain: rapid brain network reorganization in visual processing and recovery

Purpose: Although it is known that optic nerve damage, for example after glaucoma or optic neuropathy, a local event, alters global functional connectivity networks (FCN) in the brain resting state, it is unknown if and how visual deprivation a ects the dynamics of transient and rapid brain FCN changes. The synchronization between brain regions is essential for the integration between visual and non-visual modalities in time and space, and if a patient detects - or fails to detect - visual stimuli is rather variable and may depend on the FCN response to visual stimuli.

Methods: In patients with optic nerve damage (n=19) and healthy subjects (n=14), the ability to detect super-threshold stimuli was related to parameters of the ”event related network analysis (ERNA) based on graph theory immediately following successful (hits) or unsuccessful stimulus detections (misses). Graph-based features of transient and dynamically synchronized networks where described following stimulus onset to compare di erent visual field states of normal and partially damaged visual field sectors (areas of residual vision, ARVs).

Results: Compared to controls, hits in the intact visual field sector in patients where associated with connectivity topology changes characterized by less cluster, but more large scale connections with low e ciency. In areas of residual vision, hits in patients evoked a network dynamic change with weaker node strength and less clustering, shorter characteristic path length and poorer small-world-ness than hits in their intact field. These rapid FCN topology changes happened primarily in high alpha and beta band in the late ”cognitive processing stage (300-600 ms).

Conclusion: Patients with optic nerve damage have a weaker processing balance of functional integration and segregation during the cognition which reduces local and global information interactions. FCN fluctuations are thus a physiological correlate of response variability of visual functions and network modulation might be a possible target for modulating visual performance.

Research and Development of a Single Photon Counting "In Vivo Cam” for Diagnosis

This collaborative project aims the research and development of an innovative, highly sensitive, in vivo camera for diagnosis of eye disorders. A prototype of this single photon counting camera (LINCam) has been developed by our collaborators from Photonscore GmbH. This camera is able to detect auto-fluorescence in live cells in vitro with very low light intensity (<50 mW/cm²) and without any additional labelling of the cells. These preliminary observations are very promising for our aim to detect eye disorders in rats and patients in vivo through fluorescence lifetime imaging by time-correlated single-photon counting (FLIM) as a very mild procedure. In order to employ this technique in vivo, we would like to benefit from our experience in ‘in vivo confocal neuroimaging’ (ICON). This well-established method was first described by Sabel et al. Nature Medicine, 1997 and can be used to detect pre-labelled retinal ganglion cells in narcotised rats. Initial comparison of both techniques will help us to determine parameters for in vivo imaging optimisation with the new camera. Therefore, subcellular changes need to be identified, the survival of sensitive cells such as neurons needs to be monitored and long term imaging effects need to be defined under normal and pathological conditions. Further development of a user friendly software tool will finally lead to the production of an EYECam prototype, which should not just be usable for basic research on eye structures in animals, but also as prototype for an eye diagnosis system usable for future patients.

In vivo visualization different kinetic of active compound encapsulated in PLGA nanoparticles at the blood-retinal barrier

US Food and Drug administration (FDA) approved product poly (lactic-co-glycolic acid) nanoparticles (PLGA NP) have a huge potential as drug delivery systems, for imaging and diagnostic methods. Here, with In vivo Confocal Neuroimaging (ICON) we visualize in real time the biodistribution of fluorescent nanoparticles in vessels of the retina by microscopic evaluation of the distribution of the fluorescence. When working with Rhodamine123 (Rho123) labelled PLGA NPs we observed disappearance of the fluorescence within the first 15 minutes after injection. However, with 1,1’-dioctadecyl-3,3,3’,3’-tetramethylindocarbocyanine perchlorate (DiI) labelled poly (lactic-co-glycolic acid) nanoparticles showed more long-lasting e ects. The final result showed that fluorescent signal of the hydrophobic marker DiI can last for more than 1.5 hours in blood vessels which are significantly longer than for the hydrophilic Rho123, although Rho123 as well as DiI-labelled PLGA NP were clearly visible a significant fluorescent signal in the retina vessels from shortly after injection up to approximately 5 min later. In the case of Rho123-labelled PLGA NP application, no fluorescent was detectable at later time-points 15 min, but clearly visible fluorescent lining of the vessels can be seen for more than 2 hours after injection of DiI-loaded PLGA NP. By quantification of the fluorescent signal in the retina blood vessel we created a temporal-spatial map of the active ingredients distribution. With this work we contribute to a better understanding of the causal relationship between design of nanoparticulate carrier systems and their distribution at the blood-retinal barrier (BRB), which will be helpful for future drug development projects for the treatment of optic nerve damage, for example after glaucoma or optic neuropathy.

Microsaccades, vision restoration and brain network reorganization in glaucoma patients and effects of EYE YOGA therapy

Understanding how the human brain reversibly generates and loses normal vision, through a complex interaction of neural activity at multiple spatial and temporal scales, is a grand challenge for modern neuroscience. Recent theoretical advances have argued that vision deteriorates when the balance between integrated and differentiated neural activity is affected. However, accurately tracking these changes in brain dynamics remains a key research challenge with potentially wide-ranging applications, and is complicated by the significant individual variability in the trajectory along which vision is deteriorated and alleviated.

We propose to study the effects of EYE YOGA, which is a "low tech but high concept” solution with an enormous potential to revolutionize visual rehabilitation by offering an alternative, behavioral technique that is easy to use in the patients´ home environment. We expect that this very simple technique which everyone can practice at home (even normally sighted people) can help improve vision related function. By way of reducing the tension eye muscle and facial tissues, synchronizing brain network interactions, and by possibly improving blood flow, we expect EYE YOGA to be established as an ancient, yet novel and very effective vision rehabilitation method to improve vision by reducing fogginess and increasing acuity.

This project will help change the mind of the research and clinical community. It is time to start looking at mechanisms of vision loss beyond the eye such as brain network plasticity. We should consider follow a more holistic approaches to treat low vision and blindness by making use of modern technology in combination with ancient methods of healing.

Prediction of vision recovery rate after stroke based brain graph network and deep neural networks

This multidisciplinary project draws from the fields of neurology, informatics and medical engineering research to develop a new method for the prediction and diagnostics of visual dysfunctions after visual system damage. The final goal is to find methods to improve vision after optic nerve damage, for example after glaucoma or optic neuropathy, and for stroke. About 1/3 of all stroke patients’ su er posterior artery territory damage which leads to visual impairments (hemianopia) which decreases of life quality. Less is known about the mechanism of how brain works with the neurons which managed to survive and how the brain could recover and which kinds of treatments are useful. According to the "residual vision activation theory”, visual functions can in part be activated and restored because some residual structures are usually spared after damage. EEG is an electrophysiological monitoring method to record electrical activity of the brain. Brain stimulation was a typically noninvasive common method to treat the brain injuries for lot of clinical applications, here 24 patients were assigned into three groups and accepted the brain stimulation therapy for ten days, resting state EEG data was recorded while patients kept eyes closed in a no task condition, the data was preprocessed and resourced into a 3D brain model, brain connectivity were analyzed on power and phase as well as the correlation with HRP data, the di erent areas will be marked for next step machine learning. Deep neural network (deep learning) can allow us to gain lots of insight based on its high performance with undefined features. Therefore, we combine the deep learning technology and brain graph network to make prediction how the brain recovers following brain stimulation treatment. Generally, this topic would be highlighted by the integrated technologies such brain imaging and deep learning, the result could be referred as an alternatively way to help the stroke patients in their daily life.

Microsaccade alteration in primary open-angle glaucoma and the effects in treatment induced by noninvasive transorbital Alternating Current Stimulation for vision restoration

Glaucoma is a progressive optic neuropathy associated with degeneration of retinal ganglion cells and their axons, which has an impact on complex daily behaviors, such as driving, walking, and eye-hand coordination, which may be affected by visual field defects (VFDs). Little is known about the underlying nature of the functional mechanisms influencing impairment from VFDs, but it seems likely that worsening of eye movements may play a role in this process.
Microsaccades (MS) are a kind of fixational eye movement. They play a significant role in counteracting foveal and peripheral fading. MS can be measured precisely with reliable parameters in recent years. Because of their important role in normal vision, it is reasonable to assume that MS are also affected in glaucoma patients.
Noninvasive, repetitive transorbital alternating current stimulation (rtACS), as a new method to enhance neuronal function both in normal subjects and in patients with some residual vision, was proved effective and safe in the treatment of glaucoma. Based on the theory about the role of MS in vision fixation it was of importance to investigate that whether MS could be one of the significant cause of the efficacy of vision restoration as induced by rtACS.
Therefore, the aim of this study will be to determine whether microsaccadic eye movements are altered in glaucoma patients and whether there are changes of MS-related parameters during the treatment for vision restoration induced by rtACS.

Polymeric nanoparticles targeting the CNS: between the new PVP NPs crossing the BBB and siRNA- PBCA NPs for glaucoma treatment

In the last two decades, polymeric nanoparticles such PBCA, PLGA and PLA NPs have been extensively studied for brain drug delivery due to their biodegradable and biocompatible proprieties. We now studied polymeric NPs produced from Poly-vinyl-pyrrolidone (PVP NPs) as a new nano-carrier-system and studied their ability to pass the blood-brain-barrier (BBB) after systemic administration.

Using the blood-retina-barrier (BRB) as a surrogate of the BBB, we utilized in-vivo confocal neuroimaging (ICON) for live retinal imaging and compared our results with ex-vivo wholemount retina preparation. By loading NPs with fluorescent agents and using double/triple fluorescent labeling protocols for ex-vivo wholemount retinae, we were able to observe the distribution of NPs in the vessels and the parenchyma of the retina.

PVP NPs loaded with 1, 1'-dioctadecyl-3, 3, 3', 3'-tetramethylindocarbocyanine perchlorate (DIL), a substitute for hydrophobic drugs, were found to be able to cross the BRB when linked with 5(6)-carboxyfluorescein diacetate N-succinimidyl ester (CFSE) which accumulated in retinal tissues within 10 min after injection.

Apart from the PVP NPs, PBCA NPs was also used to study the inhibition of retinal ganglion cells apoptosis when loaded with caspase-3-siRNA after intravitreal injection. The cells survival was tracked for 21 and 41 days post- optic nerve crush (ONC) also using ICON as it allows repetitive real-time imaging for the same rat. The results have shown a lower cell death after treatment (-35%) in comparison with the control group (-56%). Furthermore, ex-vivo wholemount retina has been successfully performed to study the RGCs morphology changes on higher cellular level with better magnification.

Psychosomatic factors in the vision restoration
Laufzeit: 01.09.2016 bis 31.12.2020

Objective: To study whether psychosomatic factors including age, gender, personality traits, chronic stress levels and Flammer syndrome signs a ect the degree of vision restoration in patients with glaucoma or non-glaucoma vision loss.

Methods: Total of 30 patients with glaucoma or non-glaucoma vision loss, aged from 20 to 86 years old are asked to complete psychological questionnaires: The NEO Five-Factor Inventory-3 (NEO-FFI), Trier Inventory for Chronic Stress (TICS), Type D scale (DS-14) and Flammer syndrome questionnaires within two years after the application of repetitive transorbital alternating current stimulation (rtACS) 10 days in SAVIR-Center.

The study will unveil if age, gender and chronic stress influence vision restoration. We expect that patient's recovery better if they have less neuroticism and more conscientiousness and openness. Also, the Flammer syndrome might have a negative influence on visual restoration of visual field index in the worse eye.

Drug delivery with polybutylcyanoacrylate nanoparticles to the retina, brain and main organs of rats

Because the blood-brain barrier (BBB) is an obstacle for drug-delivery, carrier systems such as polybutylcyanoacrylate (PBCA) nanoparticles (NPs) have been studied. Yet, little is known of how physiochemical features such as size, surfactants and surface charge influence BBB passage in vivo. We used a rat model of in vivo imaging of the retina - which is brain tissue and can reflect the situation at the BBB - to study how size and surface charge determine NPs ability to cross the blood-retina barrier (BRB). The result showed that for poloxamer 188-modified, DEAE-dextran-stabilized PBCA NPs, decreasing the average zeta-size from 272 nm to 172 nm by centrifugation reduced the BRB passage of the NPs substantially. Varying the zeta potential within the narrow range of 0 15 mV by adding di erent amounts of stabilizer revealed that 0 mV and 15 mV were less desirable than 5 mV which facilitated the BRB passage. Then we removed and imaged the retina of the rats ex vivo to observe the detailed location of the NPs in retina tissue. Similar as the in vivo result, the NPs with larger zeta-size and 5 mV surface charge accumulated more in the vessel wall and in retina ganglion cells. Interestingly, the NPs with 0 mV surface charge accumulated unevenly in vessel wall and some agglomerates attached on the surface of the vessel wall. We also collected blood, brain, heart, kidneys, liver, lungs and spleen of the rats. The biological distribution of NPs in blood and brain is comparable to the results of in vivo imaging of blood vessel and retina tissue. Thus, minor changes in design of nanocarriers can alter physicochemical parameters such as size or zeta potential, thus substantially influencing NPs biological distribution in vivo.

REVIS: Restitution von Sehleistungen nach Schlaganfall durch nicht-invasive elektrische Hirnstimulation (ERA-NET NEURON)

Der Forschungsverbund "REVIS" (Restoration of Vision after Stroke) befasst sich mit der Plastizität des visuellen Systems und der Evaluation eines nicht-invasiven elektrischen Hirnstimulationsverfahrens zur Restitution von Sehleistungen. Weltweit gibt es 11 Mio. Schlaganfall-Patienten, die aufgrund von Sehbeeinträchtigungen erhebliche Alltagsprobleme erfahren (pro Jahr 2,1 Mio. Neufälle). Über eine Stärkung der neuronalen Plastizität mittels nicht-invasiver Elektrostimulation könnte die Sehleistung in Gebieten des Residualsehens bei Patienten mit Gesichtsfelddefekten nach Posteriorinfarkten verbessert werden. Mit einem breiten Methodenspektrum werden zunächst visuelle Dysfunktionen identifiziert und schließlich Merkmale der Postläsionsplastizität nach Hirnstimulation (Reorganisation rezeptiver Felder, lokale Aktivierung und Konnektivitäten) dokumentiert. Wechsel- oder Gleichstrom-Stimulation (supraorbital bzw. transkranial) sollen Veränderungen der lokalen und globalen Plastizität bewirken und dadurch eine deutliche Verbesserung der Sehleistung ermöglichen, die Alltagsrelevanz haben und zu einer verbesserten sehbezogenen Lebensqualität führen (z.B. Orientierung im Raum und Leseleistung). Mit dem Verbundprojekt soll somit einerseits ein besseres Verständnis der neuronalen Mechanismen (Plastizität des Gehirns) erreicht und andererseits den betroffenen Patienten eine mögliche Therapie der Gesichtsfelddefekte angeboten werden. "REVIS" wird in Magdeburg koordiniert (B. Sabel / C. Gall, Institut für Medizinische Psychologie; O. Speck Institut für Experimentelle Physik, Magdeburg) und Partner sind P. Rossini (Rom), T. Tatlisumak (Helsinki) sowie - für Tierstudien - V. Waleszczyk (Warschau).

Veränderungen der rhythmischen Hirnaktivität und visuelle Funktionserholung nach non-invasiver transorbitaler Wechselstromstimulation
Laufzeit: 01.06.2010 bis 31.03.2015

Nicht-invasive Wechselstromstimulation (tACS) kann den Rhythmus der Gehirnaktivität bei gesunden Probanden beeinflussen und zu Verbesserungen von Gesichtsfelddefekten bei Patienten mit Schädigung des Nervus opticus führen. Es ist unklar, ob Veränderungen der Gehirnaktivität mit der funktionellen Verbesserung des Sehens nach tACS zusammenhängen. Um dies zu prüfen, werden beide Parameter in Beziehung gesetzt und Veränderungen des EEGs über den Behandlungszeitraum zwischen tACS-Patienten und Placebo-Patienten verglichen. Methoden des Data Mining werden eingesetzt, um beispielsweise systematische Erhöhungen der Alpha-Power während der Therapiesitzungen zu untersuchen.

7-Tesla Bildgebung der Residualleistungen und Plastizität des Sehsystems nach Schlaganfall
Laufzeit: 01.07.2009 bis 31.12.2012

Ziel des Projektes ist es, mittels hochauflösender Bildgebung an einem 7T-Magnetresonanz-tomographen bei hirngeschädigten Patienten mit partiellen Läsionen des visuellen Kortex perimetrisch identifizierte teilgeschädigte Hirnareale direkt sichtbar zu machen und diese funktionell zu reizen. Dazu wird zunächst mit Hilfe etablierter Reizmuster der visuelle Kortex retinotop kartiert und das Gesichtsfeld mittels Elektroretinogramm elektrophysiologisch charakterisiert. Anschließend werden durch Computersimulation Gebiete des Residualsehens perimetrisch identifiziert und die Gesichtsfeldkoordinaten in kortikale Koordinaten transformiert. Dies erlaubt es, spezielle Sehreize zur Aktivierung der teilgeschädigten Areale zu entwerfen. Diese Reize werden unter Kontrolle der Augenbewegungen in den Gebieten des Residualsehens sowie spiegelbildlich auf der gegenüberliegenden, intakten Seite präsentiert und fMRT-Aktivierungsmuster quantifiziert. Als Kontrollgruppe dienen gesunde, altersgematchte Probanden. 7T-Bildgebung erfolgt vor und nach non-invasiver transorbitaler Wechselstromstimulation, die mit dem Ziel der Reduktion der Gesichtsfelddefekte eingesetzt wird.

Der Einfluss des BDNF Polymorphismus auf die Effektivität restorativer Methoden zur Gesichtsfelderweiterung
Laufzeit: 01.07.2009 bis 31.12.2012

Gehirnplastizität wird unter anderem auch von genetischen Gegebenheiten beeinflusst. Ziel des Forschungsvorhabens ist daher zu prüfen, ob der Brain-derived neurotrophic factor (BDNF) einen modulierenden Einfluss auf die Restitution des Gesichtsfeldes haben kann. Dies ist möglich, da BDNF nachweislich bei synaptischer Plastizität, Wachstum von Axonen und Dendriten, sowie Überleben von Zellen in verschiedenen Hirnregionen eine Rolle spielt. Auch im visuellen System hat dieses Protein seine Bedeutung beim Schutz gegen lichtinduzierte Photorezeptordegeneration, der Regulation der zellulären Verfeinerung und Verzweigung der Dendriten der Netzhautganglienzellen, sowie der Modulierung der Augendominanz. Zudem wurde festgestellt, dass Elektrostimulation und visueller Input die Expression und Funktionalität des BDNF unter Umständen beeinflussen können. Der BDNF scheint somit einen potentiellen Einfluss auf Restitutionsprozesse im visuellen System ausüben zu können. Wenn sich dies bewahrheitet, würde jedes Hindernis, das die optimale Funktionalität des Proteins beeinträchtigt (z.B. Polymorphismus), in einem limitierten Therapieerfolg der visuellen Restitutionstherapie und der non-invasiven Wechselstromstimulation resultieren.

Transcorneale elektrische Stimulation (RTACS) zur Restitution/Protektion visueller Funktionen im Tiermodell: RTACS, EEG-, EEP- und VEP-Aufzeichnungen in der frei-beweglichen Ratte
Laufzeit: 01.09.2011 bis 31.12.2011

In Kooperation mit einem Kollegen von der Moskauer Universität haben wir ein Setup entwickelt mit dem Stimulationen und Aufzeichnung an nicht-narkotisierten Tieren durchgeführt werden können. Es wurde ein Sockelaufbau entwickelt, mit dem Elektroden in verschiedenen Arealen des visuellen Systems platziert werden können und das EEG der Ratte freibeweglich abgeleitet werden kann. Die Flash-light Stimulation erfolgt über eine weitere head-stage. Dadurch kann auch das VEP (visuell evozierte Potentiale) an der nicht-narkotisierten Ratte ermittelt werden. Durch technische Veränderung bei der Applikation der Stimulation ist es uns jetzt auch möglich, direkt nach Gabe des Wechselstrompulses das elektrisch evozierte Potential (EEP) zu messen. Ein weiteres Ziel ist, über diesen Aufbau den applizierten Strom zu bestimmen.

Innovative methods in neuropsychological rehabilitation

The project focuses on neuropsychological rehabilitation of patients that have suffered brain stroke or trauma. Brain injuries often lead to loss or impairment of functions, like language, memory, attention, vision, hearing, emotional or motor control, etc. which are usually accompanied by personality disturbances and/or impairments in patients daily activities. Epidemiological studies show a worldwide increase in the incidence of brain-damage and besides the enormous magnitude of human suffering, brain disorders are a large economic burden to society due to the associated direct and indirect costs. Estimates of such cost are in the range of many billion EUROs and the DANA ALLIANCES in New York has estimated that one third of all medical costs can be attributed to CNS disorders. A primary goal of modern neuroscience is therefore to find new ways to treat neurological and psychiatric diseases more effectively. The recent developments of neuroimaging diagnostic methods (such as e.g., functional or structural magnetic resonance imaging, positron emission tomography, magneto-encephalography, electrophysiology) permits the identification of brain regions involved in particular mental tasks, pointing clinicians and researchers to the complexity of interactions in brain injury repair and recovery of function. In spite of many approaches to mapping the working brain, our knowledge on mechanisms underlying recovery of functions are still relatively limited. Professional intervention programs (to satisfy patients needs in a relatively short time) and our ability to meet economic demands constitute important priorities of modern rehabilitation. These priorities point to important factors beyond patients control. Increasing evidence suggests that rehabilitation programs are clinically effective and can improve cognitive functioning.

In Vivo Confocal Neuroimaging (ICON): Neue Perspektiven für die nicht-invasive Echtzeit-Bildgebung im Zentralnervensystem der Säugetiere
Laufzeit: 01.04.2006 bis 31.03.2011

Die Untersuchung der Struktur des zentralen Nervensystems, d. h. die Darstellung von Zellen wie Neuronen oder Glia, erfordert eine mikroskopische Darstellung von histologisch präpariertem Gewebe, das zu einem bestimmten Zeitpunkt - etwa nach einer Läsion - dem Tier entnommen wird. Die Darstellung von zeitlichen Veränderungen war bisher nur durch Zeitreihenanalysen möglich, bei denen für jeden einzelnen Zeitpunkt separate Tiere benötigt wurden. Dadurch konnten Populations- und Funktionsanalysen normaler und degenerierender Nervenzellen bisher nur im statistischen Vergleich zwischen Individuen durchgeführt werden, mit dem Nachteil interindividueller Streuung und hoher Versuchstierzahl. Auch ist keine Echtzeitanalyse der Zellmorphologie des ZNS in vivo möglich. Die Dokumentation des zeitlichen Verlauf der neuronalen Degeneration ist aber für ein besseres Verständnis der Gesamtproblematik zwingend erforderlich. Mit der Methode des in vivo Imaging retinaler Ganglienzellen mit konfokaler Mikroskopie ist es möglich die degenerativen Veränderungen fluoreszenzmarkierter RGC zu beobachten. Dies bedeutet einen großen Vorteil gegenüber konventionellen Methoden, da der Zeitverlauf im Individuum und in Echtzeit beobachtet werden kann. Somit liegt ein Werkzeug zur nicht-invasiven Beobachtung von Nervenzellen im lebenden Tier vor, dass nicht nur im Bereich der Degenerations- oder Retinaforschung, sondern insgesamt im Bereich der Neurobiologie Anwendung finden kann. Die gewonnen Erkenntnisse (anatomisch, funktionell, molekularbiologisch) sind lediglich von den eingesetzten Fluoreszenzmarkern und ihrer Eigenschaft (struktur- oder funktionsabhängig) abhängig, denn ICON ist nicht an morphologische Marker gebunden. Alle fluoreszierenden und zellverträglichen Marker sind prinzipiell einsetzbar und geben entsprechend ihren Transport- und Fluoreszenzeigenschaften Auskunft über spezielle Aspekte der untersuchten retinalen Ganglienzellen. Mit ICON können so RGCs wiederholt im selben Tier über einen Zeitraum von Wochen und Monaten ausgezählt und vermessen werden (Anzahl, Größe und Fluoreszenzintensität der RGC), ohne dass durch die Laserbestrahlung Zellen zu Schaden kommen. Das ICON-Verfahren hat dabei für die Zellerkennung im Vergleich zu herkömmlichen Verfahren der retinalen Flachpräparat-Histologie eine vergleichbar hohe Effizienz von 96%.

Elektrovisuelle Stimulation: Verfahren zur neurologischen Rehabilitation von Wahrnehmungsstörungen in Patienten mit Teilerblindung
Laufzeit: 01.07.2008 bis 31.12.2010

Ziel ist es ein neues Elektrostimulationsverfahren zu entwickeln, welches durch die Stärkung des Residualsehens zu einer Verbesserung des Sehvermögens von teilerblindeten Patienten führt. Hierbei soll eine nicht-invasive elektrovisuelle Stimulation die residualen Strukturen des primären visuellen Systems gereizt werden, so dass durch Synchronisation eine anhaltende Aktivitätssteigerung und synaptische Plastizität erreicht wird. Molekular werden durch repetetive Salven von schwachen Elektroimpulsen trophische Faktoren freigesetzt, die eine Stärkung der synaptischen Übertragung zur Folge haben.

In vivo Untersuchung von Nanopartikelformulierungen mittels ICON
Laufzeit: 01.01.2008 bis 31.12.2010

Biologische Barrieren stellen für die professionelle Pharmaentwicklung oft ein unüberwindbares Problem dar, da häufig Substanzen, insbesondere Gene, die sich in vitro als therapeutisch äußerst wirksam erweisen, für die Anwendung in vivo dennoch nicht geeignet sind, wenn sie biologische Barrieren nicht oder nur unzureichend überwinden können. Daher besteht ein aktuelles Ziel der pharmazeutischen Industrie darin, Methoden zu suchen, mit deren Hilfe Wirkstoffe in Zellen des Körpers eingeschleust werden können. Auf Basis dieser Problematik wird die ICON-Methode (In Vivo Confocal Neuroimaging) als Standardprüfverfahren für die in vivo Nanopartikelforschung eingesetzt. Erste Ergebnisse zeigen, dass das ICON-Verfahren für Untersuchungen zum Einsatz polymerer Nanopartikel als Trägersystem für Wirkstoffe besonders gut geeignet ist. ICON hat gegenüber Untersuchungen am toten Tier eine sehr viel höhere räumliche und zeitliche Auflösung. Mittels einer am Laser Scanning Mikroskop angeschlossenen Kamera können zusätzlich real-time Filmsequenzen aufgenommen werden. Dies garantiert eine noch bessere zeitliche Auflösung des Übertritts von Partikeln über die Blut-Retina-Schranke.

Wiederherstellung von Sprachfunktionen durch transkranielle Wechselstromstimulation bei Aphasiepatienten
Laufzeit: 01.05.2009 bis 31.01.2010

Im Rahmen der Studie werden aphasische Störungen nach linkshemisphärischer Schädigung des Gehirns mit paraorbital applizierter, transkranieller Wechselstromstimulation (tACS) behandelt. Eine Aphasie zeichnet sich durch komplexe Sprachstörungen aus, die sowohl das Verstehen von Sprache als auch deren Produktion betreffen. Für die Untersuchung der Effekte der Wechselstromstimulation werden zwei Gruppen von Patienten verglichen. Für beide Gruppen werden neben der Ableitung eines EEGs Aspekte ihrer Sprache sowie andere kognitive Fähigkeiten, z.B. Gedächtnis und Aufmerksamkeit, beurteilt. Neben der konventionellen Sprachtherapie erhält Gruppe 1 für 10 Tage (je 5 Wochentage mit einer 2 tägigen Pause am Wochenende) die tACS-Behandlung, während die Gruppe 2 eine Scheinstimulation erhält. Im Vergleich zu herkömmlichen Therapiemethoden wird für die tACS eine schnellere und deutlichere Verbesserung der Sprachfunktionen einerseits und andererseits weiterer kognitiver Bereiche (Gedächtnis, Aufmerksamkeit) erwartet. Es wird davon ausgegangen, dass tACS zu globalen Verbesserungen aller Sprachfunktionen im Vergleich zur herkömmlichen Therapie führt, in der jede Sprachfunktion einzeln behandelt wird.

Alkoholtoleranztest: Frühindikator von Alkoholmissbrauch bei Schülern und Jugendlichen
Laufzeit: 01.06.2007 bis 31.12.2008

Alkoholkonsum bei Schülern und Jugendlichen ist ein wachsendes Problem. Darum sind dringend präventive Maßnahmen erforderlich, um Schüler und Jugendliche schon frühzeitig auf problematisches Trinkverhalten individuell aufmerksam zu machen. Ob Problemtrinken vorliegt, kann nicht durch direkte Befragung des Alkoholkonsums entdeckt werden, da die Menge des Alkoholkonsums in der Regel bagatellisiert wird. Da aber die Alkohol -Toleranz ( wie viel können Sie vertragen? ) direkt von der Höhe und Dauer des Alkoholkonsums abhängt ( Trinktraining )  und hierüber auch gerne Auskunft gegeben wird, wollen wir einen Alkoholtoleranzfragebogen für Jugendliche entwickeln (J-MATT: Magdeburger-Alkoholtoleranz-Test für Jugendliche), um exzessives Trinken bei Jugendlichen frühzeitig zu erkennen. Die Prävention besteht nun darin, dass über eine Rückmeldung der individuellen Ergebnisse ( wo stehe ich mit meinem Alkoholkonsum, bin ich gefährdet? ) Schüler und Jugendlichen schon sehr früh auf eine mögliche Alkoholproblematik aufmerksam gemacht werden sollen. Wir wollen bei hohen Werten im J-MATT gefährdeten Jugendlichen eine Rückmeldung geben und Aufklärung anbieten. Sodann soll geprüft werden, ob sich durch diese Maßnahme der Alkoholkonsum der Jugendlichen verringert. Damit wäre J-MATT nicht nur ein Test, sondern auch ein Instrument für die Prävention.

Klinische Prüfung von VRT bei Schlaganfallpatienten
Laufzeit: 01.01.2003 bis 28.02.2007

Es wird eine kontrollierte Doppelblindstudie zur Erfassung der prospektiven Therapieeffekte von VRT durchgeführt. Es erfolgt ein Vergleich zwischenen VRT und Compensationstraining. Es werden Patienten mit Gesichtsfeldausfällen untersucht.

InnoRegio Innomed - Verbundprojekt: Definition und Erprobung wissenschaftlicher Algorithmen und Strukturen auf dem Sachgebiet der visuellen Restitution, TP 4: Empirische Verfahren
Laufzeit: 01.06.2004 bis 31.01.2006

Es sollen mit an Patienten erhobenen Daten weiterführende Untersuchungen über die Mechanismen der Gehirnreparatur durchgeführt werden. Ziel ist zum einen die Gewinnung neuer Erkentnisse, die die Basis für die Entwicklung neuer Produkte zur Behandlung von Sehstörungen liefern sollen. Zum anderen soll eine wissenschaftliche Datenbank geschaffen werden, in der alle über das visuelle Restitutionstraining erhobenen Daten international einheitlich strukturiert, aber in unterschiedlicher Weise interpretierbar, abrufbar sind.

InnoRegio Innomed - Verbundprojekt: Computergestütztes Sehtraining, TP1: Erforschung computergestützten Gesichtsfeldtrainings bei Kindern mit cerebralen Sehstörungen
Laufzeit: 01.01.2003 bis 30.09.2004

Ziel des Vorhabens ist die Neuroplastizität des kindlichen Gehirns. Anhand der Befunde der geplanten Studie sollen prognostische Faktoren für insbesondere bei Tumorerkrankungen und traumabedingten kortikalen Schädigungen bei Kindern zugrundeliegenden Restitutionsprozesse aufgezeigt werden. Zudem werden motivationsfördernde Bedingungen eines kindgerechten und indikationsspezifischen Gesichtsfeldtrainings erforscht.

Erholung visueller Funktionen in der adulten Ratte nach Schädigung des Nervus opticus
Laufzeit: 01.09.1999 bis 31.12.2002

Im Labor des Institutes konnte am Tiermodell gezeigt werden, dass sich trotz massivem Zelltod retinaler Ganglienzellen nach Schädigung einige visuelle Funktionen sehr gut vom anfänglichen Schaden erholen können.Ziel des Projektes ist es, einzelne Parameter des Sehens(Kontrastsensitivität, Sehschärfe) während der Erholung genau verfolgen zu können und dies mit dem funktionellen Status retinaler Ganglienzellen zu korrelieren. Hierzu soll das unlängst entwickelte ICON-Verfahren (In Vivo Confokales Neuroimaging) nach ONC zum Einsatz gelangen. ICON erlaubt die Darstellung der Zahl, Größe und Calcium-Aktivität einzelner RGCs in der lebenden Ratte. Von besonderem Interesse ist dabei die Lokalisation und Homöostase des intrazellulären Calciums in den überlebenden RGCs und die dynamische Veränderung nach ONC.

In Vivo-Konfokales Neuroimaging(ICON): Entwicklung eines hochauflösenden Verfahrens zur Darstellung lebender Zellen der Retina
Laufzeit: 01.01.1999 bis 31.12.2002

In den letzten Jahren wurde von uns eine neue konfokale In vivo-Mokroskopiermethode (ICON)entwickelt, um Neuronen des ZNS im lebenden Einzeltier wiederholt untersuchen zu können. Durch weitere Verbesserung der Technik, vor allem der Auflösung, der Empfindlichkeit und der eingesetzten Marker, sollen in diesem Projekt zelluläre Veränderungen noch detaillierter untersucht werden.

Prof. Dr. Bernhard A. Sabel ist seit 1992 in Magdeburg auf dem Gebiet der Neurowissenschaften tätig. Seine Forschungsarbeiten umfassen Untersuchungen neuropsychologischer und neurobiologischer Aspekte der Plastizität und Reparatur des Gehirns, mit spezieller Fragestellung der Funktionserholung nach partiellem Schaden des visuellen Systems nach Verletzungen, Glaukom oder Schlaganfall. Hauptschwerpunkte sind die Diagnose und Behandlung von Sehdefekten bei Schlaganfall- und Glaukom Patienten durch Training und nicht-invasive Elektrostimulation. Mit Hilfe dieses Verfahrens wird die Störung in der natürlichen Synchronisation der hirnelektrischen Aktivität mittels etablierter EEG-Diagnostik bei betroffenen Patienten als Veränderungen in der Hirnwellen-Aktivität (Amplitude und Frequenz) festgestellt. Zweiter Schwerpunkt ist der Einsatz polymerer Nanopartikel zur Überwindung biologischer Barrieren um Wirkstoffe in Zellen des Körpers einzuschleusen und so therapeutische Ziele zu erreichen.
